ABOUT FILING:

  • Forms must be printed on 8.5 x 14” Legal Size Paper.

  • After purchasing and printing your forms, fill them out with an ink pen. File at the Clerk of Court’s office in the parish which you are filing.

  • Do not use white-out or scratch completely through any mistakes. The Clerk of Court must be able to see what the document originally stated.

BEFORE FILING

  • In Forma Pauperis Affidavit - No Charge; If accepted, this form may allow you to defer upfront filing fees

  • All forms are made with the intention of being filed in Lafayette Parish. If you are not filing within Lafayette Parish, you may put one line through “Lafayette” and above it write in the name of the parish you are filing in above.

  • If you are NOT a U.S. citizen, you should call the Clerk of Court’s office in the parish you intend to file to confirm that you are able to file forms before proceeding.

  • If you have NOT lived in Lafayette, Acadia or Vermilion Parish for at least 6 months, you should call the Clerk of Court’s office in the parish you intend to file to confirm that you are able to file forms before proceeding.
