Should you purchase the wrong form or make a mistake while filling out a form, the Lafayette Bar will allow you to exchange your form for either a new copy of the same form or for another form of equal or lesser value. There are absolutely NO refunds. To exchange your forms you:

• MUST come into the Lafayette Bar office during operating hours

• MUST have the original copy of the form

• MUST have your receipt that shows proof of purchase

If you make a mistake filling out your forms, you are allowed to draw one line through the incorrect information and write above or below with the correct information. DO NOT use white-out or scratch completely through.
